Key Responsibilities:

Manage a caseload of customers with varying levels of complexity and risk
Conduct specialist assessments in homes to determine eligibility and risk
Recommend and arrange equipment and adaptations to promote safe, independent living
Design and specify cost-effective home adaptations, including scale drawings
Prescribe wheelchairs and accessories in line with NHS guidance
Provide emotional support to customers and carers during major environmental changes
Complete re-housing assessments and compile detailed reports
Carry out moving and handling risk assessments and provide training to carers and professionals

What We're Looking For:

A qualified Occupational Therapist registered with HCPC
Experience working with individuals with physical, mental, or learning disabilities
Strong assessment and care planning skills
Ability to work collaboratively with health, housing, and voluntary sector partners
Commitment to safeguarding and promoting the welfare of vulnerable adults
