Through integration and collaboration, we'rebuilding a health system underpinned by partnership, equity, sustainability,whānau-centred care, and excellence.**About the role**We have an exciting opportunity for aninnovative Mātanga Tapuhi Nurse Practitioner (NP) Health of Older People with apassion for Community based practice.Thiscross-sector role based atHutt Hospital improves access to comprehensive gerontology assessment (CGA) and coordinated interventions to support older adults to live safelyin their own homes. To improve equity, the Mātanga Tapuhi NP will work with Māori and Pacific people aged 55 + years whopresent to the Community ORA team withdiagnoses usually associated with older people that impact their function andsocial well-being.One of the keyobjectives of our wider service is to support older adults to live safely intheir own homes as long as possible. The NP role will contribute to thisservice objective by providing atimely, flexible and collaborative service with links to primary care and ourcommunity based agency partners.The NP will assist with identifying acute or emergent issuesand offering treatment guidance and care to prevent further deterioration orloss of function. We are looking for an autonomous practitioner but you willnot be working in isolation as the role is embedded with the Community OlderAdults, Rehab and Allied health team whilst working closely with ourGeriatricians and Frailty Registrar.The district is alsoconsidering a range of approaches designed to prevent avoidable EDpresentations and support hospital flow. So you may be involved in Residential Transitional care or otherevolving initiatives.**About our team**Te Herenga Tangata is our Community Older Adults, Rehabilitationand Allied Health service. We are alarge, friendly, interdisciplinary team offering you access toPhysiotherapists, along with Social Workers, Occupational Therapists, Nurses,Speech Language therapists, and Dietitians colleagues. Across the broaderCommunity Older Peoples service there are other NPs and Senior Nurses. You will also have supportfrom the Nurse Director and linkage with the Nurse Consultant AdvancedPractice. Given that we are now part ofa single District with Capital Coast you will also be linking with NPs workingwithin the CC Community ORA teams.We are continuing tobuild relationships with Primary Health and a broad range of government and communityproviders. This collaboration is already making it easier for our staff toprovide timelier, effective and equity focused health services. You will be involved in the ongoing evolutionof these collaborative partnerships as we develop our practice to respond tothe diversity of the Hutt Valley.**About you**We wish to see applications from people who are passionate about thehealth of our Older Adults population and committed to models of practice thatwill support the wider health system.
