# Registered Nurse - Early Discharge and Rehabilitation Service at Health New Zealand - Te Whatu Ora Southern, Otago## Mō tēnei tūranga mahi | About this role**Registered Nurse – Early Discharge and Rehabilitation Service (EDRS)****Salary band $81,000 - $106,000 + KiwiSaver*** **Dunedin Hospital*** **Full time 1.0 FTE / 80 hours per fortnight, Part-time role considered*** **Permanent vacancy**Health New Zealand | Te Whatu Ora is firmly grounded in the principles of Te Tiriti o Waitangi and is committed to building a health system that serves all New Zealanders.**About the role**In the Early Discharge and Rehabilitation Service (EDRS), you'll become part of a friendly, experienced interdisciplinary team that values collaboration, innovation and delivering outstanding care.You'll enjoy the autonomy of community nursing while being well supported by colleagues who are passionate about rehabilitation and helping people remain safely at home.This is a rewarding opportunity to work alongside physiotherapists, occupational therapists, rehabilitation assistants, psychologists, social workers and older persons health teams including geriatricians and CNS's to support patients as they transition from hospital back into the community.Our team provides assessment, rehabilitation and clinical support within patients' homes and aged residential care facilities, helping people regain independence, avoid unnecessary hospital admissions and achieve the goals that matter most to them.Success comes from building strong working relationships across the health system, including with inpatient teams, primary care, aged residential care providers and community services, ensuring patients experience seamless, coordinated care.Key responsibilities include:* Providing comprehensive nursing assessment and clinical support for patients receiving rehabilitation in the community.* Monitoring patients receiving rehabilitation programmes and care packages, identifying changes in clinical status and responding appropriately.* Working closely with the interdisciplinary team to develop and deliver individualised rehabilitation plans.* Building collaborative relationships with inpatient services, General Practice, aged residential care facilities and other community providers to support smooth transitions of care.* Delegating and supporting Rehabilitation Assistants as part of delivering high-quality rehabilitation.* Contributing to service development and continuous quality improvement.**About us**Health New Zealand I Te Whatu Ora - Southern has the largest geographical coverage of all the New Zealand regions, providing services to 350,000 people in diverse communities.
