Job Description

About the Role We are seeking an enthusiastic and experienced Addictions Case Manager, who is either an allied health professional or registered nurse with experience in addiction or mental health settings. Responsibilities Provide comprehensive assessment, care coordination, treatment planning and recovery-focused interventions for individuals experiencing substance use and co‑existing mental health concerns. Facilitate medicated withdrawal where appropriate and support clients through withdrawal and recovery. Collaborate with clients, whānau, and support networks to promote freedom from addiction, enhance wellbeing and empower them to live fulfilling lives. Coordinate with a multidisciplinary team of clinicians and social service providers to deliver high‑quality care. Maintain accurate clinical documentation and manage a varied caseload. Promote resilience, empowerment and partnership within a recovery‑oriented framework. Qualifications Current New Zealand Annual Practising Certificate in relevant professional discipline, preferably with an addiction focus, or a working pathway to an addiction‑specific qualification. Strong ability to provide high‑quality assessment, intervention and care coordination within a recovery‑focused framework. Commitment to delivering responsive, holistic, person‑centred care that supports individuals and their whānau to achieve recovery goals. Understanding of contemporary addiction and mental health practice, including harm minimisation and recovery principles. Ability to work independently and as part of a multidisciplinary team. Commitment to cultural safety, equity and improving health outcomes for diverse populations. Excellent organisational skills, sound clinical judgement and the ability to manage a varied caseload. About the Team, Service and Location The service provides specialist community‑based addiction and mental health support for individuals experiencing substance use challenges and co‑existing mental health concerns. You will be joining a supportive and experienced multidisciplinary team comprising clinicians from a range of professional backgrounds committed to delivering high‑quality addiction and mental health services in Nelson Marlborough.
