Buller District - Reefton - Integrated Family Health Centre (IFHC) Permanent part‑time position working 48 hours per fortnight (0.6FTE). Shared rostered on‑call. About the Role As a valued member of our interprofessional team, you will deliver a broad range of primary care, urgent care, and community services within the Integrated Family Health Centre, as well as community‑based care through home visits. You will support the health and wellbeing of tamariki, adults, and whānau by delivering culturally responsive, holistic care to our rural communities. Key Responsibilities Deliver high‑quality care within primary care and community settings to enhance health and wellbeing outcomes. Provide palliative care, wound care and medication management in the community. Provide culturally competent care, prioritising the needs of Māori as tangata whenua to achieve equity in health outcomes. Undertake comprehensive, holistic assessments within culturally appropriate frameworks. Develop collaborative and documented care plans that support continuity of care. Work closely with community stakeholders and the interprofessional team to address unmet healthcare needs. Foster integration of acute, community, mental health, and primary care within the IFHS. Apply critical thinking to support a culture of safety, effectiveness, and patient‑centred care. Work independently as well as collaboratively to ensure the highest standard of patient care. Maintain professional, legal, and ethical responsibilities while ensuring cultural safety. Identify opportunities for service enhancement and support quality improvement initiatives. Qualifications Essential Requirements New Zealand Registered Nurse with a current Annual Practising Certificate (APC) without conditions that may impact role performance. Strong initiative and ability to think on your feet. PRIME trained or commitment to completing training and providing this service to the community. Desirable Qualifications & Experience Computer literacy, including experience with Indici and Microsoft Office Suite. Previous experience as a Practice Nurse or District Nurse, including managing long‑term conditions and working with standing orders. Current Immunisation and Cervical Smear Certificates.
