This is a full-time permanent role, with weekend work on a rostered basis.In this role, you will work alongside a multidisciplinary team (consisting of Registered Nurses, Occupational Therapists, Physiotherapists and other Allied Health Assistants) to deliver interventions and rehabilitation plans with patients. This role is varied and involves supporting patients in their own homes to achieve their goals.**About you** You will have experience in rehabilitation and a natural empathy for people.
