Te whiwhinga mahi: The opportunity We have an amazing opportunity to join our team as a Clinical Diabetes Specialist, to work across the primary care sector in Taranaki. The role will provide clinical expertise and support to the Diabetes Integrated Team to deliver care to people with diabetes (PWD) and their whānau, in line with the model of integrated care whilst meeting the MOH Quality Standards for Diabetes Care. The role will work collaboratively across partner organisations from multiple locations including the hospital environment, community and within general practice clinics. You will provide targeted support, guidance, education and advice to the healthcare workforce. Your aim is to build a confident workforce in their management of PWD, enabling more patients to be cared for safely and to feel empowered in alignment with guidelines and best practice. Wherever possible doing sowithin a primary and community care setting, closer to home. You will promote and role model excellence in diabetes management including change processes to support the Diabetes Integrated Team. You will champion clinical excellence, training and education to support workforce development of diabetes management in Pinnacle practices across the Taranaki region. This role i focuses on improving workforce competency in the management and delivery of diabetes care services within general practices and supporting primary care nurses / Allied health to increase knowledge, skills and competence against relevant national standards and frameworks. As the sole clinical educator for this service, demonstrated confidence and the ability to deliver quality clinical education for primary care on diabetes management is considered a core competency for this role. Your creativity will be utilised to increase engagement with priority group populations including innovative community-based activity. You will have excellent interpersonal, skills, problem-solving abilities and the ability to challenge, inspire, influence, enable and encourage others. This is a permanent full-time position, working Monday to Friday 40 hours per week. There are aspects of evening clinics and weekend community support where flexibility within your 40 hours is expected by negotiation. He kōrero mōu: About you A registered clinician with current practicing certificate. Designated prescriber. Hold a postgraduate in diabetes education. (Preferable) Have a full and current driver’s licence. Have a minimum three-year health experience in primary care. Have experience in diabetes management. Have experience in patient and staff education. Show initiative and innovation in engaging with broader health services.
