About the Role We are seeking an experienced and passionate Team Co-ordinator to lead and support our Health of Older People (HOP) team within Support Net, delivering essential Needs Assessment and Service Coordination (NASC) services to our community. This is a unique opportunity to combine clinical expertise, operational leadership, and people development in a role that directly impacts the wellbeing and independence of older people and individuals with disabilities. As Team Co-ordinator, you will: Provide day-to-day operational leadership and coordination of NASC services Support and coach Needs Assessors and Service Coordinators Maintain a small clinical caseload to stay connected to practice Ensure high-quality, person-centred, and culturally safe service delivery Work closely with the Regional Team Leader to deliver service excellence This role sits at the heart of service delivery balancing people leadership, clinical oversight, and operational performance. Support Net is part of the Regional Community team based in the eastern Bay of Plenty. The Health of Older People team works in collaboration with kaumātua, whānau, community providers and other health professionals to ensure safe, effective care plans are in place. About you Relevant qualification in health, disability, or social services (e.g., nursing, allied health) Current NZ Annual Practising Certificate Strong understanding of Te Tiriti o Waitangi and its application in practice Experience & Skills: Leadership or team coordination experience Strong communication and stakeholder engagement skills Ability to manage competing priorities and lead in a dynamic environment Commitment to equity, culturally safe practice, and community-led care Highly desirable: Experience in NASC / needs assessment InterRAI experience Knowledge of the aged care and disability sector Working at Health New Zealand Health New Zealand | Te Whatu Ora is dedicated to ensuring excellent healthcare for the people of New Zealand.
