Job Description

About the Role As a Registered Health Professional (Social Worker or Occupational Therapist) you will play a key role in supporting older people and their whānau through their mental health and recovery journey. Undertake comprehensive assessments and develop tailored care or rehabilitation plans. Deliver interventions that promote independence, wellbeing, and quality of life. Support people living with dementia in both community and residential care settings. Work collaboratively with whānau, carers, and other providers to improve outcomes. Contribute to meaningful, person‑centred goals through therapeutic engagement and rehabilitation. Provide care coordination and support functional rehabilitation and environmental adaptations. Work as part of a collaborative, multidisciplinary team to deliver safe, effective, and integrated care. About you Relevant qualification in Social Work or Occupational Therapy with current NZ registration and APC. Experience in mental health, ideally in community or older persons services. Strong assessment, clinical reasoning, and intervention planning skills. Confidence working within a multidisciplinary team and building effective relationships. Full NZ driver’s licence. Commitment to culturally safe and equitable practice.
