About the Role Permanent Full time - 0.9 FTE We have an exciting opportunity for a Nurse Practitioner to join our Renal Service, where your emerging expertise will make a tangible difference in the lives of patients across the Waitematā district. You will provide comprehensive advanced assessment, clinical expertise, and leadership for patients with renal disease that will enhance patient treatment and care planning, whilst actively engaging with family/whanau to promote best health outcomes for patients in Te Whatu Ora Waitematā. Specifically, this role will provide day to day medical cover for our community dialysis units, including the afternoon and evening patients, providing day to day review of presenting issues, assessment of dry weights, assessment of vascular access, review of monthly bloods and six-monthly reviews. About the Team / Service / Location This role will be based at our Community Dialysis Units. You will be working within an interdisciplinary model, across the continuum of care to best meet patient needs, in line with the vision and values of Waitematā Health.
