Role Description The Community Support Worker will provide home‑based support to clients in the Nelson area, helping them maintain independence and quality of life. This part‑time, on‑site role involves assisting with personal care, household tasks, and community activities, following individualised care plans and professional guidelines. Day‑to‑day tasks include supporting clients with daily living activities, accompanying them to appointments or social outings, monitoring changes in their health or wellbeing, and promptly reporting concerns to the clinical team. The Community Support Worker will maintain accurate documentation, communicate effectively with clients, whānau/families, and other healthcare providers, and contribute to a safe, respectful, and culturally responsive care environment. Qualifications Experience in community support, home care, or disability support services, with a commitment to person‑centred care. Strong communication and interpersonal skills, including the ability to build trust and work respectfully with people from diverse backgrounds. Reliable time management and organisational skills, with the ability to follow care plans and service procedures accurately. Basic computer and documentation skills to record client notes and communicate with the wider healthcare team. Relevant health or support‑related qualification (e.g., NZ Certificate in Health and Wellbeing or equivalent) or a willingness to work towards one. Understanding of safe care practices, including infection prevention, manual handling, and confidentiality requirements. Ability to work independently in clients’ homes, with sound judgement and a responsible, compassionate approach. Current full or restricted driver’s licence and access to reliable transport for travel within the Nelson area.
