Job Description
Te Kuuwaha o te Waiora 135C Silverdale Road, Hillcrest, Hamilton Permanent – 80 hours per fortnight About the Role The ACT team provides intensive and flexible support to Tangata Whaiora presenting with complexity. The multidisciplinary team collaborates across services to ensure wrap‑around support that meets the needs of Tangata Whaiora and their whanau. Caseloads are fluid and indicative of the complexity of care required at any time. Working Arrangements 7 day a week service, inclusive of public holidays (08:00 – 16:30 hrs) Early evening on‑call Monday to Friday (16:30 – 20:30 hrs); on‑call clinicians may need to return to work for urgent client related business Self‑rostering to support work/life balance Responsibilities Provide comprehensive clinical support to Tangata Whaiora with complex needs Assess, formulate presentations and risks, and report to the multidisciplinary team (MDT) in a clear and concise manner Engage therapeutically, employ critical thinking, and communicate effectively with clients, whanau, and the MDT Collaborate with allied health, social work and other disciplines to deliver integrated care Maintain accurate and timely clinical documentation in line with organisational policy Qualifications Current annual practising certificate enabling work in the mental health sector Experience within the mental health field Strong interpersonal, therapeutic engagement and communication skills Ability to work autonomously within a multidisciplinary team and focus on service user/tangata whaiora needs Good understanding of community services and resources available to clients Knowledge and understanding of Te Tiriti o Waitangi Current full New Zealand driver’s licence Inclusive Candidate Statement Health New Zealand | Te Whatu Ora welcomes applications from our diverse Māori, Pacific, disabled, and rainbow communities.
