Location: Hutt Community Contract: Permanent, Full Time (1 FTE) About the role The Primary Care Liaison Service offers a Nurse Practitioner (NP) position (or NP Candidacy) to enhance multidisciplinary care for people with mental health needs. The role focuses on clinical leadership, consultation with primary care and NGO sectors, and ensuring timely and community‑based care for tangata whaiora. For current Capital Coast and Hutt Valley Health NZ employees, the NP Candidacy period will be a secondment. Salary: Senior Nursing Grade 6 as Nurse Practitioner (NP). For NP Candidacy the salary is Senior Nursing Grade 3. Responsibilities Provide assessment, treatment, and transition support within secondary and primary mental health services. Collaborate with primary care providers to maximise treatment and support continuity of care. Provide clinical leadership and consultation to meet identified health priorities. Engage in continuing education and up‑skilling initiatives across the sector. Qualifications & experience Registered Nurse (RN) with no restrictions from the Nursing Council of New Zealand and a current practising certificate. Minimum 3 years experience in mental health nursing. Expert level on the PDRP or Senior Expert pathway with at least 3–4 years relevant expertise in an advanced RN practice level. Clinical Masters or comparable postgraduate courses required to undertake the NP postgraduate practicum in alignment with the national Nurse Practitioner Training Programme (NPTP). Experience in quality improvement and clinical processes. Strong interpersonal and relationship‑building skills, high self‑awareness, and reflection. Team / Service location The Primary Care Liaison Service teams cover the Hutt Valley region and the wider MHAIDS area, working closely with primary care settings to provide high‑quality mental health assessment and to facilitate seamless transition between primary and secondary care.
