Job Description
Job Title Registered Nurse – START Taupo (Permanent, Part time – 48 hours a fortnight, Rostered) Overview This unique opportunity allows you to move beyond the traditional hospital setting and make a tangible difference in patients' lives where it matters most: their own homes. As part of the Supported Transfer and Accelerated Rehab Team (START), you will play a key role in patients' rehabilitation journeys, helping them regain independence and confidence in a familiar environment. Responsibilities Lead clinical nursing efforts within START to streamline discharge processes and enhance patient rehabilitation. Provide intensive, holistic support that enables medically cleared patients to return home sooner through individualized rehabilitation plans. Travel across the Lakes district to deliver care in patients' homes, focusing on assessment, planning, delivery, and evaluation of safe, appropriate nursing care. Supervise and direct Health Care Assistants (HCAs) and coordinate shifts as needed. Conduct complex clinical assessments and provide cross‑cover support to District Nursing. Work collaboratively with a multidisciplinary team (MDT) to adjust care plans, manage resources cost‑effectively, and ensure a seamless patient journey across the care continuum. Qualifications & Expertise Hold relevant graduate and post‑graduate qualifications; operate strictly within the RN scope of practice and NCNZ competencies. Proven ability to lead and supervise others, with the critical thinking skills required to manage clinical escalations and complex ethical or legal concerns. Deep understanding of Te Tiriti o Waitangi in action, with a drive to achieve equitable health outcomes for Māori and a critical awareness of how your own culture impacts service delivery. Excellent communication skills; can build rapport with whānau and collaborate effectively with GPs, allied health staff, and the wider LICHS team. Demonstrated adaptability and resilience; able to work in a roaming, community‑based role, manage stress, and solve problems independently.
