As part of a close-knit team of six administrators, you will play a key role in coordinating services that support people to remain safe, well, and independent within their communities.***What you will be doing:**** Facilitating service arrangements with community-based providers* Responding to phone and email enquiries* Completing accurate and timely data entry and referral processing* Liaising with clients, family/whānau, health professionals, and external agencies* Supporting the referral, assessment, and service coordination processes for community and residential support servicesThis is a busy and varied role. You'll have the opportunity to work alongside experienced clinical and administrative professionals, develop your knowledge of community health services, and contribute meaningfully to positive outcomes for people across Otago. We provide a broad range of administrative support functions for the Care Coordination Centre, including processing referrals, data entry, and working across a range of electronic systems such as SIPICS, interRAI, and Health Connect South. We also respond to a variety of enquiries via phone and email from clients, whānau, health professionals, and community providers.We pride ourselves on fostering a positive, collaborative team environment.
