Job Description

Health Care Assistant – START Taupo Position type: Permanent, Part‑time (48 hours a fortnight) – Rostered. Location: Taupo. This is a unique opportunity to move beyond the traditional hospital setting and make a tangible difference in patients' lives from the comfort of their own homes. As part of the Supported Transfer and Accelerated Rehab Team (START) you will provide high‑level personal care, support rehabilitation exercises, and document progress while working autonomously as a key member of the recovery team. Responsibilities Travel across the Lakes District to visit patients recently discharged from hospital. Assist patients with daily living activities such as mobility exercises, personal care and meal preparation. Follow the specific rehabilitation plan designed by the clinical team. Monitor and record patient progress, reporting observations to Physiotherapists, Occupational Therapists and Registered Nurses. Ensure patients feel safe, supported and confident in their recovery journey. About the Team / Service / Location The START team is a multidisciplinary group dedicated to the Early Supported Discharge model, providing intensive short‑term rehabilitation at home to prevent hospital readmission. Qualifications & Experience Previous experience in a healthcare setting (hospital, aged‑care or home care) with a focus on rehabilitation or patient support. Excellent interpersonal skills and the ability to build rapport quickly with patients and their whānau in their own homes. High level of initiative and the ability to work independently while following clinical instructions. Requirements Clean, full NewZealand driver’s licence. Benefits Competitive salary.
