Benefits Supportive and culturally grounded team environment Opportunities for professional growth and development Meaningful and impactful work supporting Māori health equity Flexible, community based work Wellness programmes and staff support initiatives Responsibilities Work primarily in the community to support whānau Māori with complex health needs Prevent avoidable hospital admissions, reduce readmissions, and improve discharge outcomes Coordinate care across primary, secondary and community services Collaborate with iwi, Māori health providers and wider health services to provide early intervention and prevention Contribute to improved equitable health outcomes, strengthen discharge planning and continuity of care, reduce pressure on hospital services and support Pae Ora outcomes Qualifications Current NZ Registered Nurse APC (without conditions) Full NZ Driver's Licence Experience working in community or complex care settings Strong ability to build relationships with whānau and community providers Commitment to Te Tiriti o Waitangi and improving Māori health outcomes Confidence working independently and across services Team & Service Pae Ora Paiaka Whaiora sits within the Māori Health Directorate and works closely with iwi partners, community providers and hospital services to deliver whānau‑centred care.
