Clinical Nurse Specialist – Gerontology, Medical and Elder Services, Whangarei Hospital (Permanent part‑time position, 0.8FTE, 32 hours per week) Health New Zealand | Te Whatu Ora is firmly grounded in the principles of Te Tiriti o Waitangi and is committed to building a health system that serves all New Zealanders. About the Role As a Clinical Nurse Specialist, you will provide advanced clinical expertise, leadership and education to support the delivery of high‑quality, person‑centred care for older adults at Whangarei Hospital. You will work collaboratively within a multidisciplinary team to improve patient outcomes, promote best practice and support service development. Key Indicators Undertake assessment and interventions with elder clients to prevent admissions, support early discharge and maintain clients in their own environment. Be part of the Acute Community Elder specialist nursing team, working with Acute Care of the Elder ED GNS, nurse practitioner, geriatricians and clinic Enrolled Nurse. Participate in strengthening and developing community specialist support models through Aged Residential Care. Work with wider primary and community care services to facilitate integrated services for elder people, supporting optimisation of elder clients in their own environment, home and assistance with avoiding unnecessary hospital admissions. Participate in developing pathways to provide timely access to specialist nursing and medical advice (Dementia pathway, ACP, falls). About the Team / Service / Location Join a supportive, experienced Health of Older People team committed to excellence in elder care and make a real difference in the lives of older adults across our community. This role is based at Whangarei Hospital and is part‑time working 32 hours rostered and includes some weekend working. It is located in Northland. About You Registered with the Nursing Council of New Zealand and a current Annual Practicing Certificate (APC). Current Expert Registered Nurse Professional Development and Recognition Programme (PDRP) portfolio. Postgraduate diploma in specialty nursing practice, working towards or willing to undertake. Specialty knowledge and practise expertise. A personal commitment to ongoing learning and development. Communicates information effectively and accurately, both orally and in writing.
