Clinical Needs Assessor – Health of Older People Fixed Term, Part-time (up to 12 months, 40 hours per fortnight – 0.5 FTE, Monday to Friday, 7:00 AM to 5:00 PM, no on‑call) About the Role You will support the wider Needs Assessment team with service co‑ordination activity and communication processes, and work closely with Wairau Hospital, other health agencies, community services and Iwi/Health providers. The role requires a commitment to a restorative model and experience working with older adults with a health‑related disability or those under the age of 65 affected by complex health conditions. Responsibilities Support coordination of services and communication processes for the Needs Assessment team. Build and maintain strong relationships with disability, health, and community services, providers and funders. Engage with older people, their families and caregivers, ensuring their needs and aspirations remain central to planning and decision‑making. Collaborate with the team to deliver outcomes for people we serve. Qualifications and Experience Experienced Clinical Needs Assessor or similar role. Experience working with individuals with disabilities or older adults. Knowledge of disability models of care and support. Full driver’s licence. Ability to work independently and as part of a team. Flexible and adaptable to changing priorities. Allied health background preferred. Eligible to work in New Zealand or holding a valid NZ visa. Location Blenheim Health Hub, Marlborough.
