Aboutthe Role Wehave an exciting opportunity to join the Diabetes Specialist Service as aDiabetes Clinical Nurse Specialist. Our team are dedicated to empowering peopleto manage their Diabetes. The 0.6 FTE permanent position offers a role in aclinical leadership position to shape strategic direction and deliver a holistic level of care. Asa senior member of the team, you will focus on and embrace connectivity by: Working with communities to build, maintain and enablestrong foundations for health and wellbeing of patients to manage theirDiabetes. Prioritise disease prevention, health promotion andhealth & wellbeing. Understand the needs of patients and communities,enabling them to exercise authority over their health and wellbeing. Grow and continually develop the Diabetes service andits care delivery to a high standard of care. AboutYou A Registered Nurse with at least five years' diabetesnursing experience or relevant nursing role. You will be an expert at leading and developing nursingpractice to improve health outcomes and the management of Diabetes. Excellent interpersonal skills and ability to buildeffective relationships with a strong patient focus. A current APC. Ability to work collaboratively in a multidisciplinaryteam. Relevant postgraduate qualifications, a nurseprescriber or willing to obtain a nurse prescribing qualification. Comfortable with technology and its use to supportself-management and improve health outcomes.
