Clinical Needs Assessor - Health of Older People

The Community Services Team values teamwork and is seeking a motivated team member to join us. In this role you will work alongside our Wairau Hospital Assessor building strong relationships with disability, health and community services, as well as community providers and funders. Responsibilities Support the wider Needs Assessment team with service coordination activity and communication processes. Maintain close working relationships with Wairau Hospital, other health agencies, community services and iwi/health providers. Work within a restorative model and collaborate with older people with a health‑related disability or those under 65 affected by complex health conditions. Facilitate service coordination and communications for the team. Qualifications and Experience Experience working with individuals with disabilities, including older adults or those with complex health conditions. Knowledge and understanding of disability models of care and support. Ability to work both independently and as part of a team. Full driver’s licence. Preference for an allied health background. Eligibility to work in NewZealand or hold a valid NZ visa. Employment Details Employment type: Permanent part‑time (0.5FTE). Hours: 40hours per fortnight (Monday to Friday, 07:00AM to 05:00PM).
