Job Description

Lead and grow The Fono’s outreach services, providing strategic leadership to multidisciplinary teams delivering integrated, whānau centred care. This role drives health equity outcomes, service innovation, and community impact across Auckland. The Fono is a Pacific-led provider of health and community services, and plays a pivotal role in improving health equity for Pacific, Māori and other priority communities. As Outreach Manager, you will provide strategic and operational leadership across our outreach services in Henderson, Manurewa and Blockhouse Bay, leading multidisciplinary teams to deliver integrated, community-centred care. This senior leadership role is responsible for driving service growth, achieving contract outcomes, strengthening partnerships, and developing innovative systems and reporting frameworks that demonstrate impact and support continuous improvement. We are seeking an experienced healthcare leader with a strong background in primary or community health, proven success managing multidisciplinary teams, and a passion for culturally responsive, whānau-centred service delivery. What you will do Provide strategic and operational leadership across The Fono’s outreach services, ensuring programmes are aligned, effective, and responsive to community needs. Lead and support multidisciplinary outreach teams to deliver integrated, whānau-centred care. Champion health equity by improving access, engagement, and outcomes for Pacific, Māori, and other priority populations. Drive service innovation, growth, and continuous improvement initiatives across outreach programmes. Develop and implement systems, reporting frameworks, and performance measures that demonstrate service impact and support informed decision-making. Build and maintain strong relationships with Health NZ, ProCare, community partners, and key stakeholders. Monitor contract performance, service outcomes, budgets, and operational targets to ensure organisational objectives are achieved. Embody The Fono’s values of whānau relationships, reliability, respect, and integrity in all aspects of leadership and service delivery. About you Minimum 5 years' senior leadership experience in primary healthcare, community health, integrated care, or PHO environments. Experience leading multidisciplinary teams. Experience managing contract performance and KPIs. Strong understanding of Pacific and Māori health equity. Budget and operational management experience. Current Full NZ Driver Licence. Highly desirable Postgraduate qualification in Health Management, Public Health, or Leadership. Pacific cultural competency and/or Pacific language. Experience working with Health NZ, PHOs, and community providers. Experience establishing reporting and performance frameworks. #J-18808-Ljbffr
