About the Role Casual As a Nurse Practitioner, you will be part of an energetic and friendly Hospital-in-the-Home service, delivering acute care in a community-based setting. This is a unique opportunity to work autonomously while also being supported by SMO collegial guidance, alongside a highly skilled nursing and administration team. The continued expansion of the service, backed by a regional governance platform, makes this an exciting time to join and contribute to innovative models of care. In your day-to-day role, you will conduct comprehensive physical assessments, diagnose and manage acute and sub‑acute conditions, and prescribe medications to support patients at risk of deterioration. You will provide one‑on‑one care and advice, requiring strong communication, compassion, and proactive clinical decision‑making. The role also includes telehealth delivery, staff education and support, and active involvement in service improvement and policy development. About the Team/Service/Location Our Hospital‑in‑the‑Home team provides acute hospital level care to patients in their own homes, supporting better outcomes and patient experiences.
