Mō te tūnga | About the role We are seeking a Clinical Coordination Lead to provide clinical expertise and high-quality, client-centered coordination for community nursing services across Aotearoa. This role supports safe and timely entry to service, clinical triage, allocation support, and coordination through to discharge. You will work closely with Clinical Nurse Leaders, Community Nurses, the National Referral Centre, administration teams, referrers and funders to support safe service delivery, quality improvement and positive outcomes for tāngata and whānau. You will be responsible for: Providing day‑to‑day clinical triage and making timely, clinically appropriate decisions at entry to service Coordinating clinical administration from referral through to discharge, including documentation, prescriptions and consumable requirements Supporting allocation and rescheduling decisions where clinical oversight or input is required Contributing to contractual, compliance and reporting requirements, including quality activities and service improvement Building effective relationships with funders, referrers, internal teams, tāngata and whānau Ngā pūkenga me ngā wheako | What we’re looking for Registered health professional with a current Annual Practicing Certificate Minimum of five years’ relevant clinical experience Knowledge of ACC Nursing Services preferred Proven leadership, coaching and service improvement capability Strong verbal and written communication skills, with confidence using digital systems Full or restricted New Zealand driver license Ngā tohungatanga mō tēnei tūranga | Desired skills and experience Postgraduate qualification in a relevant area preferred Experience supporting complex or high‑needs clients and applying sound clinical judgement Ability to analyse information and contribute to quality reporting, audit activity and continuous improvement Demonstrated knowledge of Te Tiriti o Waitangi and commitment to culturally responsive practice Demonstrated commitment to culturally safe practice, equity, and respectful engagement with Māori, Pacific and diverse communities. Strong communication skills that support safe engagement, including awareness of health literacy and adapting communication to the person. Willingness to learn, reflect and adapt your practice in line with supervision, training, and continuous improvement. Willingness to work in this role as well as a Community Registered Nurse position where appropriate to ensure up to date knowledge of service delivery. He aha mātou e whiriwhiri ai | Why join us?
