Access Community Health is seeking a proactive Care Coordinator in Nelson to join our regional team. You’ll coordinate rosters, match workers to clients, and ensure timely, high-quality home care empowering people to live independently. You’ll work closely with clients, whānau, and Support Workers, maintaining records, handling inquiries, and escalating safety concerns when needed.
