You will Carry out comprehensive health assessments and develop individual care plans Support people to achieve their health and wellbeing goals Provide clinical care, health education, and ongoing support Deliver and support vaccination programmes Identify risks and ensure appropriate interventions are in place Coordinate care with primary, secondary, and community services Advocate for access to services and address barriers to care Support participation in preventative health initiatives and screening programmes Maintain accurate clinical documentation and meet reporting requirements This role includes both clinical care and community outreach, allowing you to build strong relationships and contribute to improved long-term health outcomes. About you You are a compassionate and motivated nurse who is committed to continuous learning and delivering high-quality, person-centred care.
