Family Harm Practitioner (Youth focused) Home and Family Charitable Trust – Christchurch, South Island Help create safer futures for rangatahi and whānau Are you passionate about supporting young people and their families to overcome the impacts of family harm? We are seeking an experienced and compassionate Family Harm Practitioner (Youth Focused) to join our Integrated Safety Response (ISR) team. This is an exciting opportunity to work alongside a multi-agency network that is committed to improving safety, wellbeing, and long-term outcomes for children, young people, and their whānau. Working within a collaborative family harm response model, you will support whānau assessed as medium to high risk, helping young people and their families navigate challenges, strengthen safety, and access the right services. About the Role As a Family Harm Practitioner, you will: Build trusting relationships with young people, whānau, and families experiencing family harm Complete safety planning and risk management activities with a strong child-centred focus Support rangatahi, their families and whānau to identify goals, strengths, and pathways to wellbeing Work collaboratively with Police, Corrections, social services, schools, youth services, and other community partners Coordinate referrals and facilitate warm handovers to appropriate specialist services Advocate for young people and families where safety concerns or barriers exist Maintain accurate case records and contribute to integrated case management planning Participate in supervision, professional development, and continuous improvement activities What You'll Bring We're looking for someone who is: Passionate about improving outcomes for young people and whānau Previous experience in family harm, youth work, social work, counselling, case management, or a related field Skilled at engaging vulnerable young people and working from a strengths-based, mana-enhancing approach Trauma-informed and child-centred in your practice Confident working alongside diverse cultures and communities Knowledgeable about the dynamics and impacts of family harm, particularly on children and young people Able to assess risk, develop safety plans, and coordinate appropriate interventions A collaborative team player who thrives in multi-agency environments Committed to ongoing learning, reflective practice, and professional development Ideally You Will Have A qualification in Social Work, Youth Work, Human Services, or a related discipline Experience working with at-risk youth, family harm, mental health, community services, or crisis intervention Understanding of Te Tiriti o Waitangi principles and confidence working within Kaupapa Māori frameworks Knowledge of local community supports and service networks A current full New Zealand driver's licence A clear result to an extensive police vetting is required for this role due to the systems access levels involved.
