Health Hawke’s Bay – Te Oranga o Te Matau-a-Māui is an established PHO, within the Hawkes Bay. We work in partnership with General Practices to improve health outcomes and life expectancy across Hawke’s Bay, particularly for Māori and other under-served groups. A Health Coach plays a vital role in general practice, supporting whaiora (patients) and whānau to build the knowledge, skills, and confidence to improve their health and wellbeing. You’ll work with whaiora in short, focused sessions to help them make practical changes that positively impact their daily lives. Using a holistic approach that values physical, mental, social, and spiritual wellbeing, you’ll work alongside whaiora to understand what matters most to them and develop achievable steps toward better health. You’ll support whaiora with: improving sleep, increasing physical activity, building healthier habits, strengthening confidence and self‑management skills, managing stress and emotional wellbeing, setting goals, and creating realistic action plans, as well as managing long‑term health conditions. Long‑term conditions may include: diabetes, heart disease or high blood pressure, asthma and other respiratory issues, obesity and metabolic concerns, chronic pain, arthritis, anxiety, depression, chronic kidney disease, and neurological or stroke‑related conditions. You’ll work closely with Health Improvement Practitioners, Community Support Workers, and clinical teams to ensure smooth and supportive care pathways, and you may also contribute to group programmes. This is a rewarding, people‑focused role where you’ll see the positive impact of your mahi on individuals and whānau every day. Ideally you will: Have a Level 4 Health and Wellbeing Certificate (completed or working towards) OR significant relevant work experience Relevant experience within the health and wellbeing sector (e.g. community health, health promotion or long-term condition management) Have experience engaging effectively with Māori and Pacific peoples Have knowledge and practical application of Te Tiriti o Waitangi, be an advocate of our Ka Hikitia strategy & willing to embrace tikangaMāori Be open to change and motivated to implement new ways of working Have good digital literacy, organisational and time management skills Learn quickly and be able to adapt to ever changing situations Be experienced in practicing autonomously Be an excellent communicator and work in a collaborative, respectful, supportive and sensitive way You will have a genuine interest in improving health outcomes for Māori and other underserved populations in our community, as well as a desire to understand and implement Māori models of care. If you are the successful candidate, you will be required to undertake mandatory Health Coach training via to be fully endorsed as a Health Coach.
