About the role We are seeking a Clinical Team Coordinator (RN) to manage andcoordinate consumer flow within the Adult inpatient group (AIG). Using expertclinical judgement, the CTC oversees referrals, admissions, transfers, andservice flow across Te Awakura, Wāhi Oranga, Wāhi Tautoko, and the widerSpecialist Mental Health Service (SMHS), ensuring tangata whaiora are placed inthe most appropriate therapeutic environment. This includes consideration ofmental health needs, risk, physical health, neurodiversity, and servicecapacity. Working in partnership with other senior nursing staff, theClinical Team Coordinator (CTC) delivers expert clinical care, advice, andleadership to support consumers/tangata whaiora and their whānau. This role is pivotal in ensuring timely access to appropriate carein the most suitable setting. It promotes continuity of care, seamlesstransitions between inpatient and community services, and strong collaborationacross SMHS teams. Through this, the CTC ensures that tangata whaiora remainsat the center of all decision-making, supporting optimal outcomes and apositive care experience. This is a (1.0FTE), Full-Time, Fixed-Term position. The rolecoversMonday-Friday 0730-1600. The position is being established on a fixed-term basis to supportthe implementation of a new model of care within the Adult Inpatient Group.During this period, the role will operate Monday to Friday to enable theservice to establish, embed, and evaluate the Clinical Team Coordinatorfunction within the service. At the completion of the establishment phase, theservice will review the role as part of the planned transition to a 24/7 modelof care, which is expected to require different coverage arrangements, hours ofwork, and staffing requirements. As the future configuration of the role hasnot yet been determined, the position is required for a limited period only. About You The Clinical Team Coordinator (CTC) is a senior nursingroleto facilitate optimum care and coordinate multi-disciplinary teamservice delivery within business hours for AIG, consumers and their families. The CTC confidently provides advanced clinicalcare,leadership and co-ordination of the MDT and co-ordination of anyemergency responses in hours, with support from other senior clinicians. Skills andexperience Acurrent NZ Nursing Registration and APC with a scope of practice thatincludes Mental Health. Recentexperience within Waitaha Specialist Mental Health Services, preferablyinpatient experience. Excellentcommunication and time management skills. Expertclinical nursing knowledge and has leadership experience. Acommitment to relevant post graduate study either having completed or havecommenced the post graduate pathway.
