About Hospice Whanganui We are a client/whānau centred service that operates 24/7, providing specialised palliative care across the Whanganui, Waimarino, and South Taranaki regions. Whether client’s are at home, in hospital, in a facility, or in our inpatient unit, we provide compassionate, expert care where they choose to be. Our experienced, specialised palliative team comprising Medical Officers, Nurse Practitioners, Nurses, Health Care Assistants and Whānau Support staff offer comfort, relief, and expert care when it is needed most. This role is fundamental to bringing whānau and services together. You will be proactive and lead challenging conversations. You will be confident to facilitate whānau hui, planning and be an integral part of the integrated team. You will also work closely with GP teams, aged care facilities, Māori health providers, the hospital team, government agencies, home based care providers, social work services, counsellors, and other community services. This can be a fast-paced role that changes daily to meet client/whānau needs. Time management and analytical skills is essential. This is a full-time position for either a registered social worker or an experienced Whānau Navigator in the health and social sectors.
