In this role, you will workalongside our Wairau Hospital Assessor building strong relationships withdisability, health, and community services, as well as community providers andfunders. About the Role You will be supporting the wider Needs Assessment team withservice co-ordination activity and communication processes, and you will have close working relationshipswith Wairau Hospital, other health agencies, community services and Iwi/Healthproviders. We will value your commitment to working within a restorative modeland your experience of working with older people with a health-relateddisability or those under the age of 65 affected by complex health conditions. This is a permanent part-time role, working 40 hours perfortnight (0.5 FTE), Monday to Friday between the hours of 7.00AM to 5.00PM. No on-call in this role. We are looking for a person who: Hasexperience working with individuals with older adults or those with disabilities; Hasthe ability to work in both a team environment as well as independently; Isflexible and adaptable. Preferenceis given for applicants with an allied health background. The successful candidate must be eligible to work in NewZealand or hold a valid NZ visa at the time of applying.
