Parish Community Nurse

Parish Community Nurse Make a measurable difference to Pacific health Tongan Health Society is seeking a culturally responsive Registered Nurse to deliver community health services through Pacific churches and community settings. You will work closely with our Clinical Services Manager, HVAZ Coordinator and 14 HVAZ Churches to improve access, strengthen prevention and connect people with timely care. The role Deliver blood pressure, blood glucose, smoking and long-term condition screening within scope and protocol. Provide individual and group health education and support healthier church and community environments. Refer and actively support people to access general practice, national screening, Whānau Ora and social services. Support breast, cervical and bowel screening participation and long-term condition self-management. Maintain timely clinical records and contribute accurate service data and reporting. You will bring New Zealand Registered Nurse status and a current Annual Practising Certificate. Community, primary care, population health or public health nursing experience. Ability to communicate effectively in a Pacific language; Tongan is highly desirable. Confidence engaging churches, community leaders, individuals and families. Strong clinical judgement, documentation skills and a full New Zealand driver licence. A recognised SME or DSME facilitator qualification, authorised vaccinator status and experience in Pacific community programmes are desirable.
