We’re looking for a hands‑on, proactive, and compassionate Clinical Community Nurse to join our growing community service delivery team. About the Role | Mō te Tūranga As a Clinical Community Nurse, you’ll be at the heart of our client care, providing direct clinical support and working closely with our resource coordinators and support workers to ensure exceptional, safe, and responsive services. This role is not desk‑bound – you will spend time out in the community, actively engaging with clients, families/whānau, and staff. From clinical assessments and care planning to training, incident management, and medication oversight, you’ll play a vital role in helping clients better understand their injuries and care needs so they can be empowered participants in their own wellbeing. Responsibilities Carry out comprehensive clinical assessments for new and existing clients. Develop, review, and maintain care plans tailored to client needs. Provide hands on clinical support in the community. Manage and investigate clinical incidents, including medication errors and pressure injuries. Deliver training, coaching, and mentoring to support workers. Work closely with clients and whanau to resolve clinical issues and enhance outcomes. Collaborate with GPs, funders, and allied health providers to ensure seamless care. Support continuous improvement and quality initiatives within tlc4u2. Requirements Current New Zealand Registered Nurse (RN) practicing certificate. At least 3 years of clinical experience, ideally in community or disability care. Strong knowledge of medication management, pressure injury care, and clinical risk management. Excellent communication skills and the ability to build strong relationships. A passion for mentoring and empowering others, including clients and support workers. A full, clean New Zealand driver’s license and willingness to travel throughout the region. Based in Northland, tlc4u2 provides specialist community‑based care for clients with complex needs, including those living with spinal cord and traumatic brain injuries.
