About the Role Therole sits in theForensicIntellectual Disability Service workingwithintheinpatient team.The role of the care manager is toact as the clinical manager for the care recipient(under IDCC&R Act),and to be able to direct and oversee their care withappropriateconsultationand input from other health workers while the care recipientis detained. Alongsidetheday to dayaspects of overseeing the care of the care recipient,the care manager will need to ensure the conditions in the care recipients careand rehabilitation plan are adhered to. Toliaise with identified health professionals to ensure the care andrehabilitation planremainsa living document and its content evolvesconsistent with the needs of the care recipient. Towork in a multidisciplinary team setting as a Care Manager, ensuring that allclinical steps necessary are taken toassistclients toachieveoptimalhealth,wellbeingand safety through theprovision ofappropriate assessment,interventionand educationandto achieve positive outcomes for clients. About theTeam/Service/ Location TheForensic Intellectual Disability Service is part of the Mental Health Addictionand Intellectual Disability Services. The service hasadult,youthand community services. TheIntellectual Disability Secure Service is a dynamic and supportive workplacewhere autonomous practice, research, professional development issupported. Youwill be joining a multi-disciplinary team of health professionals committed tothe delivery of gold standard care and rehabilitation to the population weserve. The team carry a wealth of knowledge andexpertiseinanumber ofareas relevant to intellectual disability, mental health, andbehaviours that challenge. We are a progressive service committed toproviding a culture of ongoing professional development for staff throughregular clinical supervision, excellent professionaldevelopmentanda recognition programme (PDRP) for nursing, regularinservicetraining and support towards external education. About you Tobe successful in this role these skills and experience are expected: ExperiencedRegisteredHealth Professional; RegisteredNurse, Social Worker,OccupationalTherapistorPsychologistwith a currentpractising certificate. Experienceworking with Inpatient and Community Mental Healthand IntellectualDisabilityServices. The appointeetothe positionof 'Care Manager'willneed to have theknowledge, skills and attituderequiredto allow the Care Coordinatortodesignatethem as the Care Manager under the terms of the Act forspecified care recipients and proposed care recipients. Iscomfortable operating within a complex environment, including meeting the needsof multiple stakeholders at the same time. Haveexcellent communications skills both verbal and written. Have theability tolead meetings and client reviews. Have a current Driver's License. Works well with complexity and ambiguity. Works well under pressure - keeps abalancedperspective. The knowledge requiredby a Care Manager to manage the job appropriately can be seen as covering threeoverlapping areas. These are: An understanding ofthe legal framework within which theyoperate. An understanding ofthe legal process. An understanding ofkey features of bio psychosocial variables related to Intellectual Disabilityand personality functioning.
