Location: Porirua, but expected to work acrossHutt Valley and capital Coast area. About the Role MentalHealth Needs Assessment Service Coordination is a dynamic and responsivespecialist mental health service that keep tangata whaiora at the centre ofwhat we do. We are part of the Mental Health Addictions and IntellectualDisability Service Operations Centre that covers the Capital, Coast and HuttValley district. The role is to comprehensively assess mentalhealth and addictions support needs, for people aged 18 and over and refer themon to services to work alongside them to enhance their recovery. These services may include community-basedsupports, home based supports and housing and recovery residential support. About you Skills and experience needed for the role include: New ZealandRegistered Social Worker A current AnnualPracticing Certificate (APC) and scope appropriate to place of work Expectation of atleast 5 years clinical experience Member ofAotearoa New Zealand Association of Social Workers - Professional Association (desirable) Demonstrates anunderstanding of the significance of and obligations under Te Tiriti oWaitangi, including how to apply Te Tiriti principles ina meaningful way in the social work role A commitment toachieving equitable outcomes for Māori Strong experiencein leading and delivering comprehensive assessments, and working incollaboration with people and their whānau to ensure bestpossible health and wellbeing outcomes Experience inusing critical thinking and judgement on how best to allocate resources tosupport the recommendations from the assessment Record of workingsuccessfully within complex systems to collaborate and facilitate positive outcomes A high standard of written and spoken English Full driver'slicence.
