The Doctors Silverstream is seeking a Health Improvement Practitioner to join the general practice team on a permanent part-time basis, up to 32 hours per week. The role focuses on increasing access to wellbeing and behavioural health services within primary care, working with individuals and families to support mental health and overall wellbeing.
