Are you passionate about helping people make positive changes to their health and wellbeing? The Doctors Apollo is seeking a motivated and compassionate Health Coach to join our team and support patients on their journey towards better health outcomes. are based within GP clinics, working alongside GPs, nurses, and the wider primary care team to support patients as part of their everyday care. You will help patients build the knowledge, skills and confidence they need to manage their own health and wellbeing, while connecting them with the services and support available within their community. About the Role: This is a permanent part time role, working 24 - 28 hours per week over 3-5 days, to work in clinic. Days and hours can be discussed with the successful candidate. As a Health Coaches, you will: Deliver brief, structured health coaching as part of routine primary care Support patients and their families/whanau to manage long term health conditions by building confidence and skills to better manage health Support patients to identify and achieve their own health and wellbeing goals Build and maintain strong relationships with your clinic Work alongside Health Improvement Practitioners (HIPs) and another Health Coach and a wider multi-disciplinary team Connect patients with appropriate health, wellbeing and community support services As a core member of the practice team, you will provide support through face-to-face, telephone and virtual consultations. You may also facilitate group programmes, assist patients to navigate health and wellbeing services, and work collaboratively with the wider clinical team to improve engagement and outcomes for patients who may be harder to reach. About You: We are looking for someone who genuinely enjoys working with people and is passionate about empowering others to improve their health and wellbeing. The successful applicant will have: Excellent communication and interpersonal skills A positive, motivating and non-judgemental approach The ability to connect with people from a wide range of backgrounds and cultures Good time management Genuine empathy and willingness to support people to work on their health Good organisational and administration skills Experience in skills of problem solving, self-management, goal setting and creating action plans Ability to work both independently and as part of a team Legal rights to work in New Zealand Self-motivated and confidence in reaching out to patients Culturally aware, and aware of the New Zealand health context Experience working in health, community, social services, wellbeing, fitness, nutrition or a related field would be advantageous, but is not essential. Korean language skills would be advantageous but not essential Applicants for this position should have NZ residency or a valid NZ work permit.
