Health NZ Counties Manukau is seeking a Community Liaison Clinical Nurse Specialist to optimise acute patient flow and coordinate transitions from hospital to community. The CNS will act as a clinical expert and navigator for patients with complex discharge needs, across acute, medical, surgical and specialty wards.
