Clinical Pharmacist, Community Services - Permanent, Part Time 0.8 FTE (64 hours a fortnight) Deliver expert medicines management and clinical reviews that directly improve patient outcomes. Work autonomously across primary care and community teams while leading quality improvement initiatives and influencing best practice. About the Role The purpose of this role is to provide expert clinical pharmacy support within community health services, working closely with primary care and multidisciplinary teams to optimise the safe and effective use of medicines. You will focus on delivering patient-centred medication reviews, improving health outcomes, supporting evidence-based prescribing, and leading quality improvement initiatives, education, and best-practice guidance across community services. About You We're looking for an experienced and collaborative Clinical Pharmacist who is passionate about improving patient outcomes through the safe and effective use of medicines. You'll be confident working autonomously, building strong relationships across primary care and community services, and providing expert clinical advice and medicines reviews. With a commitment to continuous quality improvement, evidence-based practice, and Te Tiriti o Waitangi, you'll bring strong analytical skills, a patient-centred approach, and a drive to influence positive change across the health sector. It is essential that you have the following: Registered as a Pharmacist with the Pharmacy Council of New Zealand holding a valid Annual Practising Certificate (APC) with no restrictions. Holds a relevant clinical post-graduate qualification or is working towards this, or has work experience sufficient to demonstrate general pharmaceutical knowledge in core areas and/or specialist pharmaceutical knowledge in management of long-term conditions. Experience in implementing Te Tiriti o Waitangi in action. NZ driver licence. Promotion of the quality and safe use of medicines. Critical appraisal skills and provision of medicine information. Experience and ability to build, develop, and maintain professional relationships within the health sector. Working for Health New Zealand.
