About the Role The purpose of this role is to provide expert clinical pharmacy support within community health services, working closely with primary care and multidisciplinary teams to optimise the safe and effective use of medicines. You will focus on delivering patient-centred medication reviews, improving health outcomes, supporting evidence-based prescribing, and leading quality improvement initiatives, education, and best-practice guidance across community services. About You We're looking for an experienced and collaborative Clinical Pharmacist who is passionate about improving patient outcomes through the safe and effective use of medicines. You'll be confident working autonomously, building strong relationships across primary care and community services, and providing expert clinical advice and medicines reviews. With a commitment to continuous quality improvement, evidence-based practice, and Te Tiriti o Waitangi, you'll bring strong analytical skills, a patient-centred approach, and a drive to influence positive change across the health sector. It is essential that you have the following: A Registered Pharmacist holding a current New Zealand Pharmacy Council Annual Practicing Certificate (with no adverse conditions) BPharm and Postgraduate Diploma/Certificate – or equivalent Previous hospital experience Previous experience at a Health New Zealand/Te Whatu Ora hospital or service A commitment to improving professional practice and life-long learning. 5 years previous hospital experience – preferably in New Zealand 2 years previous experience at a Health New Zealand/Te Whatu Ora hospital or service Working for Health New Zealand Health New Zealand | Te Whatu Ora is dedicated to ensuring excellent healthcare for the people of New Zealand.
