As a Health Coach / Community Support Worker, you will work within a General Practice, helping patients gain the knowledge, skills, tools, and confidence to become active participants in their own care and enabling them to reach their self-identified goals. Your role will encompass: Active participation within the General Practice team and support for building team competence in general health, mental health, and addictions. Delivery of high-quality Health Coaching services. Implement the Health Coaching model by providing self-management support through: Providing information and education. Teaching health management skills. Working with individuals, whānau, and groups. Promoting behaviour change. Teaching problem solving skills. Serving as the patient’s liaison person. Ensuring the patient understands and agrees with the health care plan. Providing cultural support. Helping patients navigate the healthcare system. To succeed in this role, you will have: Experience motivating others to achieve goals. Basic knowledge of long-term health conditions. Ability to work closely and collaboratively with key stakeholders. Proven ability to work with computers and using electronic medical record.
