Health New Zealand - Te Whatu Ora Porirua is seeking a Social Worker for Mental Health Needs Assessment Service Coordination. The role covers assessment of mental health and addictions needs for adults 18+, with referrals to community supports, home-based supports, housing and recovery residences; based in Porirua and working across Hutt Valley and Capital Coast.
