About therole We arelooking for a Case Manager with a Nursing or Allied Health background, who hasthe clinical skills required to assist people with current or ongoing mentalhealth problems and to maintain their place in the community. This rolewill be working within the community and within our integrated model of care. Our CAMHSteam is currently looking for skilled and passionate mental healthprofessional. The new role is vital to the mental health of our children andyoung people across the West Coast as you will be supporting them through theirjourney along with their whanau.This role will be working within thecommunity alongside our NGO partner Poutini Waiora and within our integratedmodel of care. There will be opportunities and support for professionaldevelopment and ongoing role development. Undertakingall components of case management, including: comprehensive assessment,risk assessment and management, recovery, and treatment planning Providingclient-centred care, utilising evidence-based treatment strategiesincluding, where appropriate, talking therapies WorkingMonday to Friday About You NZRegistered Health professional as a Nurse, Social Worker, OccupationalTherapist, Psychologist orPhysiotherapist. Excellentcommunication & relationship building skills, this is a developingrole with the opportunity to develop and enhance the service provision andpersonal practice. Passionfor child & youth mental health Flexibilityto work autonomously as well as part of a multi-disciplinary team Understandingand application of the principles of the treaty of Waitangi, with aculturally safe and appropriate approach to practice Astrong understanding of Māori health inequity and Māori population need Understandingof contemporary approaches to mental health and wellbeing Clean& full drivers licence as you will be traveling in DHB cars, this roleencompasses the whole of the Westcoast Karamea to Haast.
