About the Role Do you have excellent clinical assessment skills, wish to expand your knowledge and be part of a passionate, high functioning nursing team within the community, where no two days are the same? The Intermediate Care Team is part of the Community and Long-term Conditions Directorate and compromises of the Hospital in the Home (HiTH), to deliver acute hospital-level care in patients homes, while contributing to the growth of virtual and community-based healthcare.Rapid Community Access Team (RCAT), a short term service that provides enhanced Nursing and/or Allied Health input to support an early discharge from hospital or to prevent a hospital admission and the Interim Care Scheme (ICP) providing access to beds in Age Related Residential Care facilities (ARRC) to support patients who dont need to be in hospital, but are not yet ready to be supported in their own home.
