Te Waipuna Health, the primary care service of Te Oranganui Trust, seeks a Kaiāwhina to provide communications and coordination support to the community team. You will engage with whānau eligible for outreach services, immunisations and related primary care support. The Kaiāwhina will support timely and respectful contact with whānau and assist with patient recalls, helping the team maintain clear communication pathways so whānau can access appropriate services and receive whānau-centred care. #J-18808-Ljbffr
