Health NZ Counties Manukau is seeking a Community Liaison Clinical Nurse Specialist to optimise acute patient flow and ensure safe transitions from hospital to the community. You will act as clinical expert, liaison and navigator for patients with complex discharge needs across acute, medical, surgical and specialty wards.
