We have an exciting opportunity to join a kaupapa Māori wellness organisation dedicated to supporting whānau to achieve positive health and wellbeing outcomes. This role works alongside whānau to strengthen wellbeing, improve access to services, and provide practical support that helps them navigate their hauora journey with confidence. About the role As a Pūkenga Manaaki – Mana Tangata, you will provide whānau-centred support, advocacy, and navigation services that help individuals and families overcome barriers and access the care, programmes, and services they need. Working alongside our clinical and community teams, you will play an important role in supporting equitable health outcomes for whānau across the Motueka region. Key responsibilities include: Supporting whānau to navigate health and social services and progress their hauora plans. Collecting information to contribute to comprehensive assessments and whānau planning processes. Identifying health and wellbeing needs and facilitating referrals to appropriate internal and external services. Supporting smooth transitions through the health system, including assisting whānau to attend appointments where required. Building connections between whānau, hapū, iwi, community organisations, and support services. Supporting podiatry and GP clinic days, along other health & wellbeing programmes and services. Working collaboratively with community providers and multidisciplinary teams to support positive outcomes for whānau. Supporting outreach activities that encourage participation in breast, bowel, cervical, and HPV screening programmes. Engaging with whānau who may require additional support to access screening and preventative health services. Identifying barriers to service access and working alongside clinical and support teams to remove those barriers where possible. Maintaining accurate, timely, and respectful documentation that reflects both outcomes and whānau voice. You will bring Experience working with whānau Māori. Experience within health, wellbeing, community, or social service environments. A strong commitment to supporting whānau wellbeing and achieving positive community outcomes. Excellent communication and relationship-building skills, with the ability to engage effectively with people from diverse backgrounds. The ability to work collaboratively within multidisciplinary and community-based teams. Knowledge of, or willingness to learn, Te Ao Māori, tikanga Māori, and Māori models of health and wellbeing. Strong organisational skills, sound judgement, and professional boundaries. A tertiary qualification in health or social services is an advantage but not essential. A current Full NZ Driver Licence. A current First Aid Certificate, or willingness to obtain one. Make a meaningful difference in the lives of whānau and communities.
