The Cardio-RespiratoryIntegrated Specialist Services (CRISS) Advanced Lung Disease (ALD) team is asmall, dynamic, and innovative team with an exciting opportunity for a ClinicalNurse Specialist (CNS) with a special interest in Respiratory Nursing to jointhe service.As a positive role modelin clinical leadership, you will work collaboratively with the CRISS ALD/CF CNSteam, providing specialist advice, support, and education throughout thepatient's disease trajectory. You will partner with a range of healthprofessionals across the health sector to ensure high standards of care aremaintained and to support improved patient outcomes. In this role you will beresponsible for:

• Autonomously managing a case load of patients in the community,with Respiratory conditions such as COPD, Interstitial Lung Disease and CysticFibrosis
\n
• You Will hold Nurse Led Clinics and participate in MDT clinicsfor CF
\n
• Delivering advanced nursing assessments, specialistcardio-respiratory advice, and long-term condition management
\n
• Providing clinical leadership, education, and support throughoutthe patient's journey
\n
• Collaborating with multidisciplinary teams across the healthsector to deliver integrated, patient-centred care
\n

We are seeking anexperienced and motivated Registered Nurse with a strong background in Respiratorywho is passionate about delivering high-quality specialist care. You will haveexcellent clinical assessment, education, and communication skills, along withthe ability to engage patients, whānau, and colleagues while providing expertnursing leadership.You will be on thepostgraduate pathway, having completed or be working towards a relevantpostgraduate qualification. Experience in long-term condition management isdesirable.To be successful in thisrole you will have:

• Extensive clinical experience and a comprehensive background in RespiratoryNursing

• Advanced nursing assessment skills with the ability to providespecialist respiratory advice
\n
• Completed or be working towards a relevant postgraduate qualification
\n
• Experience in long-term condition management (desirable)
\n
• Excellent communication and relationship-building skills
\n
• Excellent case management and time management skills
\n
• Strong computer literacy
\n
• A commitment to clinical leadership and promoting excellence innursing practice

• A passion for maintaining high standards of care and improvingpatient outcomes
