Health & Wellness Coordinator | Registered Nurse (RN) Are you an experienced Health & Wellness Coordinator/Registered Nurse looking for a rewarding role where you can make a genuine difference in the lives of adults with intellectual disabilities. This is a full-time permanent role with long-term career stability. As the Health & Wellness Coordinator, you'll provide clinical leadership and health advocacy across multiple residential services. Working closely with Service Managers, support staff, families, and external healthcare providers, you'll ensure residents receive high-quality, person-centred healthcare and support. Monday - Friday 8am-5pm About the Role Provide health and wellness advice, education, and support to residents, families, and staff. Develop, implement, and review individual health and wellness plans. Coordinate GP, specialist, and allied health appointments, hospital admissions, and discharge planning. Monitor residents with complex health needs and recommend appropriate interventions. Deliver clinical coaching, education, and health-related training to support staff. Promote best practice in infection prevention, falls management, health monitoring, and safe clinical practice. Build strong relationships with families, multidisciplinary teams, and external health providers. To be successful in this role, you will have: Current New Zealand Registered Nurse (RN) registration with a valid Annual Practising Certificate (APC). Experience working with adults with intellectual disabilities or individuals with complex health needs. Strong clinical assessment and care coordination skills. Proven leadership, coaching, and mentoring experience. Excellent communication and relationship-building skills. A full New Zealand Driver Licence. Eligibility to complete a Police Vetting check.
