Job Description
To ensure that the needs of residents and their families are met, using current nursing care and best practice, as required by company policy, the ARC agreement, the Nursing Council of New Zealand, and in-line with Resident First Philosophies. Lead the Care Partner team in a fair and approachable manner, to ensure optimal teamwork is achieved, with consideration given to equal distribution of workload and Health and Safety requirements. Demonstrate confidence in clinical decision-making and seek upskilling as required. Key responsibilities Consistently demonstrate the Eden values of Innovation, Integrity, Community, Empowerment and Passion in all interactions with residents and their families, staff, management and external suppliers Ensure that Care Partner staff are performing their duties as an effective team and immediately resolve any issues which negatively impact teamwork View the facility with a 'One Team' approach and share Care Partner resources with other households/areas to ensure workload is as equal as possible across the facility Empower and educate the Care Partner Team to deliver care which is aligned with current best practice and the Resident First Philosophies Develop Care Plans which address the elimination of loneliness, helplessness and boredom, ensure a dignified care pathway for the dying as well as medical and care needs Ensure that families are updated in a timely manner on any changes to the residents condition Develop Care plans which include input from families and Subject Matter Experts such as the GP and the Clinical Nursing Management team Ensure that Care Plans are amended as required, to ensure they are up to date with current resident requirements On a monthly basis, achieve assigned targets for completion of InteRAI, Care Plans and staff performance appraisals Identify and report hazards, and coach the Care Partner team to identify and report any potential hazards About you Must have current APC Must have full working rights in New Zealand Must be available to work full rostered shifts 24/7 Aged Care experience preferable Regular and predictable attendance and punctuality Be confident to make clinically sound decisions, using the appropriate reference material Attend regular role-related upskilling, and seek ongoing professional development Act as an advocate for employee wellbeing Ensure incidents affecting residents, staff and visitors are reported and recorded within 24 hours Be aware of the location and use of the fire-fighting equipment in the area and fire evacuation procedures Ensure the care partner team are following fatigue management protocols Ensure that work is carried out in a safe manner and with minimal risk to self, residents, staff and visitors Participate in Occupational Health and Safety exercises as required Act appropriately to prevent and / or manage emergencies and disaster situations #J-18808-Ljbffr
